General Application Forms

Fleet Volunteer Position
Here is where you apply for any position and responsibility within the fleet. Please fill up all the required fields. All applications will be reviewed and answered as expendiently as possible, usually within a week. Only eligible candidates will be contacted by the appropriate division officer for a selection interview.

Thank you for offering your services to the fleet and its members.

Re-Commission Form
This form is used to re-admit a member who may already have an alias with Lotus Fleet (e.g. Old website).

New User Form
This form allows you to signify to Lotus Fleet at what capacity you would like to be involved. See the following thread for more information: http://lotusfleet.org/forums/viewtopic.php?f=2&t=625

Member Grievance Form
This form is to notify the JAG of any alleged violation of the General Orders or the Code of Conduct. It is also used to file any member's grievance or request for JAG mediation.

Post-Graduate Course Key Request Form
For those who have graduated, to go back to the academy and take more courses. You will not graduate a second time, and will not get a second diploma.

Request graduation from the Academy.

Academy Survey
Academy Survey. Help the Academy improve by giving feedback on what you liked and/or disliked.

Cadet Grievance Form
This is an anonymous form, in which to express a complaint or grievance.

Advance School Certification Request
Request Certification from the Advance Schools.

Command School Certification Request
Request Certification from Command School.

A-12 Application Form
Cadets will apply to the fleet via this form.

Supplemental forms for contributing data to RP.

Character Application
Apply for a new position or a second character, or request for reapplication to your existing position if you've been away from RP for a while.

Species Request
If you would like to create an RP character of a species that is not canon or you would like to provide details for a species that is not already detailed in our Species section of the Forums, submit this form.

Mission Submission
Submit an idea for a mission to be played out in our RP.
